Most delays SpaceX have encountered have been the product of government red tape or external forces (like lawsuits.) The US FAA license to launch the first rocket for example took an exceedingly long time to acquire, this was outside SpaceX’s control. Read more
FTS did respond - it just didn’t impact structural integrity that much since starship has autogenous pressurization. The tanks just bled slowly until the internal pressure was low enough for it to break apart.
